Providing housing and support to at risk youth and young adults

Our Approach in the Transitional Living Program

Safe & Stable Housing

Wrap-around Support

Pathways to Independence

TLP offers a home-based, residential living environment where participants have access to safe, stable housing while working toward personal and independent living goals. The program provides structure, consistency, and support while encouraging responsibility and self-sufficiency.

Participants receive individualized case management and mentoring tailored to their unique needs and goals. Support services extend beyond housing and may include life skills development, education planning, employment support, and connections to community resources that promote long-term stability.

Staff work alongside participants to help identify strengths, address barriers, and create realistic plans for the future.

The Transitional Living Program focuses on helping young adults build the skills and planning needed for lasting success. Through independence planning, shared household responsibilities, and goal-oriented support, participants prepare for stable housing, employment, and community integration.

Typical program participation ranges from 12–24 months, allowing time to build sustainable foundations for adulthood.
